突然に襲ってくる「ズキッ！」とした歯の痛み。
何が原因なのか、どうしたらいいのか、とても不安になりますよね。
歯が痛い時、ほとんどの方はむし歯を疑うかと思います。
しかし実際にはむし歯ではなくても歯の痛みを生じることもある、というのをご存知ですか？
今回は、様々な原因で起こる、歯痛とその対処法についてのお話です。
まず、歯痛は一般的に２種類に分けることができます。
「歯やお口の中が原因で起こる痛み」と「歯が原因ではない痛み」です。
それぞれどのようなものがあるのでしょうか。
【歯やお口の中が原因で起こる痛み】
１．むし歯
歯痛の代表的なもので、歯の表面について歯垢（プラーク）に、むし歯菌が棲みついて酸を出し、歯の表面を溶かして穴を開けることで痛みを伴います。
２．歯周病
歯周病菌が毒素を出すことによって、歯を支えている組織が破壊される疾患です。
重度になると、歯茎が腫れ上がったり歯を支えている骨をとかしてしまったりして、最終的には歯がグラグラして抜けてしまいます。
３．歯髄炎
歯の神経である歯髄に細菌感染が起こり、何もしなくてもズキズキする痛みが出る疾患です。
歯医者さんで歯の根っこの治療をする必要があります。
４．知覚過敏
虫歯はないが歯がしみる、歯ブラシが当たると痛いなどの症状がでます。
強い噛み締めや歯ぎしりがあったり、歯周病で歯茎が下がったりすることで根っこが露出し、象牙質に直接影響が加わることによって起こる疾患です。
歯磨き時のブラッシングが強すぎることが原因になる場合もあります。
５．咬合性外傷
歯並びなどの影響で、噛み合わせが強く当たるところに痛みが出る状態です。
噛み合わせの調整などの治療が必要になります。
６．親知らず
真ん中から8番目の一番奥の歯で、はえてくる場合もあれば歯茎に埋まったままの場合もあります。
斜めに萌えたり、水平に埋まっていたりすると、様々な痛みを引き起こします。
親知らずは磨きにくいため、虫歯や歯周病になって腫れることもあるため、早い段階で抜歯する場合もあります。
７．歯が割れている場合
歯を強く打ったり、固いものを噛んだりして、歯が割れたり欠けたりすると、神経が刺激されて痛みを生じます。
根っこに影響がある場合は抜歯をすることもあります。
【歯が原因ではない痛み】
１．急性副鼻腔炎
急性副鼻腔炎とは、風邪やアレルギー性鼻炎などが原因で起こります。
症状が悪化すると、頭痛や歯の痛みがあります。
２．三叉神経痛
三叉神経痛は、顔面の感覚を司る、三叉神経の異常が原因で、顔に痛みを感じる病気です。
顔の痛みではなく、歯のピリッとした痛みで始まることもあります。
３．群発頭痛
30代前後の男性に多く見られます。
目の奥の鋭い痛みや充血、鼻水が特徴ですが、かなりの確率で歯の痛みを伴うことがあります。
４．帯状疱疹
帯状疱疹とは、多くの場合、子どもの頃にかかった水疱瘡のウィルスが、体内に潜んでいて、抵抗力が弱った時などに形を変えて再び出てくる病気です。
顔や体にピリピリした痛みがあるのが特徴ですが、虫歯のような強い歯の痛みを伴うこともあります。
５．狭心症・心筋梗塞
狭心症や心筋梗塞の症状は胸が締めつけられるような痛みが特徴ですが、左の肩や腕、首の痛み、歯の痛みを訴えることもあります。
６．心因性疾患
口の中や、全身に疾患はなく、長期的に歯の痛みを訴える場合、うつ病などの、心因性疾患が原因となっているとがあります。
７．筋・筋膜性歯痛
日常生活の中で、食事など、口を開けたり閉じたりする筋肉に、強い緊張が続くと、歯の痛みを訴えることがあります。
このように、歯が原因でなくても、歯に痛みを感じるケースがいくつもあります。
原因がはっきりしない場合の歯の痛みが生じた場合、まずは歯科医院を受診してください。
詳しい検査をし、歯科が原因でないと判断した場合は対応していただける他科・他院をご紹介します。

When you have a toothache, most people will suspect cavities.
But did you know that you can actually have tooth pain even if you don’t have cavities?
This time, we will talk about toothache caused by various causes and how to deal with it.
First, toothache can generally be divided into two types.
“Pain caused by teeth and mouth” and “Pain not caused by teeth”.
What are each of them?
[Pain caused by teeth and mouth]
1. Caries
It is a typical toothache, and it is painful when plaque on the tooth surface is inhabited by dental caries bacteria to release acid, which melts the tooth surface and makes holes.
2. Periodontal disease
It is a disease in which the tissues that support the teeth are destroyed by the release of toxins by periodontal disease bacteria.
When it becomes severe, the gums swell and the bones that support the teeth are combed, and eventually the teeth fall out.
3. Pulpitis
It is a disease in which bacterial infection occurs in the pulp, which is the nerve of the tooth, causing throbbing pain without doing anything.
You need to have a dentist treat the roots of your teeth.
4. Hypersensitivity
There are no cavities, but symptoms such as tooth swelling and pain when hit by a toothbrush appear.
It is a disease that occurs when the roots are exposed due to strong clenching, bruxism, or the gums are lowered due to periodontal disease, and the dentin is directly affected.
It can also be caused by brushing too hard when brushing your teeth.
5. Occlusal trauma
It is a condition in which pain occurs where the bite is strongly hit due to the influence of the alignment of teeth.
Treatment such as adjustment of occlusion is required.
6. Wisdom tooth
It is the 8th innermost tooth from the middle and may come out or remain buried in the gums.
If it sprouts diagonally or is buried horizontally, it causes various pains.
Wisdom teeth are difficult to brush and can cause tooth decay and periodontal disease, which can cause swelling and may result in tooth extraction at an early stage.
7. If your teeth are cracked
If you hit your teeth hard or bite a hard object and your teeth crack or chip, your nerves will be stimulated and you will experience pain.
If the roots are affected, tooth extraction may be performed.
[Pain not caused by teeth]
1. 1. Acute sinusitis
Acute sinusitis is caused by a cold or allergic rhinitis.
When symptoms worsen, there are headaches and toothaches.
2. Trigeminal neuralgia
Trigeminal neuralgia is a disease in which the face feels pain due to abnormalities in the trigeminal nerve, which controls the sensation of the face.
It may start with a tingling sensation in the teeth rather than a pain in the face.
3. Swarm headache
It is often seen in men in their 30s.
It is characterized by sharp pain in the back of the eyes, redness, and runny nose, but there is a good chance that it will cause tooth pain.
4. Herpes zoster
Shingles is a disease in which the chickenpox virus that you had as a child often lurks in your body and reappears when your resistance weakens.
It is characterized by tingling pain in the face and body, but it can also be accompanied by strong tooth pain such as tooth decay.
5. Angina / myocardial infarction
Symptoms of angina and myocardial infarction are characterized by chest tightness, but they may also complain of pain in the left shoulder, arm, neck, and teeth.
6. Psychogenic illness
If you have no illness in your mouth or whole body and you complain of long-term tooth pain, it may be due to a psychogenic illness such as depression.
7. Myofascial toothache
In daily life, if the muscles that open and close the mouth, such as meals, continue to be tense, they may complain of tooth pain.
As you can see, there are many cases where you feel pain in your teeth, even if they are not the cause.
If you experience tooth pain for which the cause is unclear, first consult a dentist’s office.
If you do a detailed examination and determine that the cause is not dentistry, we will introduce you to other departments and hospitals that can handle it.
