抜くべき？みんなが悩む親知らず

先日、成田で行われたジムニー大会へ、応援に行ってきました！
友人のカメラマンとしてたくさん写真を撮ってきましたよ。
実は、私は岩手県のジムニー大会レディース部門で優勝したことがあります♪

私たちのお口のなかで、何かとトラブルの原因になりやすい歯が「親知らず」です。
抜くべきか？残すべきか？
悩んだことがある方も多いでしょう。

あるアンケート調査では、約4割の人が親知らずを抜いたことがあると回答しています。
実は、親知らずは人によって生え方が様々なので、一概にこうするべきとは言えないのです。
目安として、一般的に親知らずを抜いたほうが良い場合はこんなケースです。

・歯茎の炎症が繰り返されるとき
・虫歯や歯周病を招くリスクが高いとき
・歯並びを乱すリスクが高いとき

親知らずが真っ直ぐ正しく生えていて、歯磨きでしっかりと汚れを管理できる場合や、歯としてしっかり機能していて上述のようなトラブルを起こすリスクが低い場合は、抜歯する必要はありません。
また、親知らずが完全に歯茎のなかに埋まっている場合のほとんどは、抜かなくても問題がありません。

ただし、親知らずは長い時間をかけて少しずつ生えたり動いたりするため、抜歯するかしないかの判断も時間とともに変わってきます。
今は大丈夫でも、いつ何時トラブルを引き起こすか分かりませんので、不安がある方は定期的に歯医者さんでチェックを受けるようにしましょう！

The teeth that are prone to trouble in our mouths are “wisdom teeth”.
Should I pull it out? Should I leave it?
Many of you may have worried.

In one questionnaire survey, about 40% of respondents answered that they had missed wisdom.
Actually, wisdom wisdom varies depending on the person, so it cannot be said that this should be done in general.
As a guideline, this is the case when it is generally better to skip wisdom teeth.

・ When gum inflammation is repeated
・ When there is a high risk of causing tooth decay or periodontal disease
・ When there is a high risk of disturbing teeth

If the wisdom tooth grows straight and you can manage dirt well with brushing your teeth, or if it functions well as a tooth and has a low risk of causing the above problems, it is not necessary to remove the tooth.
Also, in most cases where wisdom teeth are completely buried in the gums, there is no problem even if it is not removed.

ソフトクリーム☆フッソについて

今日は虫歯予防のフッソについてお話します。

フッソは普通の歯磨き粉にも含まれておりますが、最も効果的なのは歯科医院で高濃度のフッソを定期的に塗ることです。原田歯科クリニックでは、主に定期検診の際にフッソを最後に塗布することができまして、フッソは３ヶ月に１度、定期検診のタイミングで塗布できます。初めてくるお子様などでも、初日にフッソを塗ることも可能ですので、ご連絡・ご予約の際にお申し付けください。

また、フッソを塗ることができる対象年齢ですが、歯が１本でも生えていれば、塗ることは可能です。虫歯に最もなりやすい時期は、歯が生えてから2～3年の間と言われています。ですので、歯が生えてからすぐにフッソを塗ることが重要になってきます。

歯肉の再生手術で若々しく（２）

原宿の「みのりんご」というカレー屋さんに行ってきました♪
キーマカレーがとっても美味しかったです。
店名の「りんご」にちなんで、帰りにりんご飴をプレゼントしてくれます！
辛いカレーを食べた後にはうってつけです！

前回、歯肉を再生する歯肉移植手術法の技法について、板取がご紹介しました。
その中で最も一般的な「遊離歯肉移植術」がどのように行われるのか、解説します！

遊離歯肉移植術は、大体2時間程度で終えることができます。手術の流れはこうです。

STEP1：移植部分の確認

歯の根元が露出している部分を中心に、歯肉の移植を行います。
他にも歯茎が硬く、痩せている部分も移植場所の対象となります。

STEP2：歯肉の剥離・縫合

移植する歯肉は上顎の口蓋を剥離します。必要なサイズに合わせて剥離し、
移植部分に縫合していきます。

STEP3：移植した歯肉の癒着

縫合部分は1週間ほどで癒着します。
術後の経過をみながら抜歯を行なえば、遊離歯肉移植術の完了です。

遊離歯肉移植術の手術は、麻酔を使用して行うので痛みを感じることはありません。
また、術後は移植部分がしみたり、縫合の糸が気になることもあると思いますが、術後はだいたい1週間程度で抜糸できます。

遊離歯肉移植術の手術が完了した後は、定期検診や歯磨きなどしっかり行い、大切な歯茎を守りましょう！

歯肉の再生手術で若々しく（１）

先日、衛生士の齋藤さんから辛いラーメンをもらいました！
今夜食べようと思います♪

歯を支える歯茎が細菌や加齢による影響で、痩せて衰えることがあります。
歯茎が下がると歯周病リスクが高まるだけでなく、老けて見えてしまいますが、下がった歯茎は再生手術で改善できます！

歯肉を再生する手術は、歯肉移植手術法と総称され、いくつかの技法があります。

技法1：結合組織移植術

CTG法(connective tissue graft)と言われます。
上顎の口蓋を切開して、上皮をめくりその中の結合組織を採取して移植を行います。
この技法なら歯茎が痩せて薄くなっていても、健康な厚みにまで再生させることができます。

技法2：組織再生療法

GTR法(guided tissue regeneration)と言われます。
この技法では人工膜を用いて、空洞化した歯周組織の再生を促します。
歯周組織が再生された後は、再度切開して人工膜を取り除くことになります。

技法3：遊離歯肉移植術

FGG法(free gingival graft)と言われ、CTG法と似た技法になりますが、FGG法では上皮ごと移植を行います。
数か月経つ頃には移植部分と癒着して、しっかりとした歯茎になって再生されます。

技法4：ヒアルロン酸治療

シワ改善などに使用されるヒアルロン酸を、歯茎に注入して再生を促す技法です。
ヒアルロン酸の注入で歯茎に弾力を与え、歯肉に厚みを持たせることができます。
しかし、ヒアルロン酸は体内に吸収されるため、定期的に注入する必要があります。

現在のところ、歯科で行なわれる歯茎再生手術は、遊離歯肉移植術が一般的です。

痛くないから大丈夫は間違い！

歯周病は痛みがなくても着々と進行しているものです。

実は来院された方に歯周病のお話をすると、ご自身で思っていた以上に進行していてビックリされる方が結構いらっしゃいます。
多くの方が「痛くなかったから大丈夫だと思っていた」「痛みが消えたから治ったと思っていた」と仰ることが多いです。

ここで大事なのが、歯周病は自然治癒することはありません。
歯周病は『Silent Disease(静かなる病気)』と表現されることがあり、ひどくなるまで病気と自覚されることの少ない病気です。

歯茎が腫れている状態では、歯茎は炎症を起こしてる状態なのですが、ここで重要なのが炎症の種類です。

痛みがある場合は急性炎症の状態だと思っていただければ大丈夫です。
例えば腕とか脚でこれらの症状が出ればすぐにわかりますよね？
しかしこれが歯茎だとなかなかわかりません。

慢性炎症の場合、緩やかにかつ持続的に進行している状態を指し、歯周病の症状はこれに属します。

ですから、痛みがないからといって油断せず、歯磨きの時の出血など、なにか違和感を感じたら一度歯医者さんを受診することをお勧めします。

むし歯予防にはフッ素が良い！
歯が生えたらフッ素塗布をしましょう！

…とよく言われますが、フッ素って何なんでしょう？

フッ化物とは化学的に合成されたものではなく、自然界に分布している元素です。
土壌や海水、動植物、肉・野菜・果物などの食品にも微量に含まれています。

フッ素がむし歯予防として良いとされる理由は、

・再石灰化の促進
・歯質強化
・細菌の酸産生抑制

の３つの効果があるからです。

歯の質を高める、つまり酸に強い歯にするためにフッ素はとても効果的なのです。
エナメル質の耐酸性が高くなると虫歯になりにくい歯が作られます。

ほとんどの歯磨き粉にもフッ化物が配合されています。
身近な虫歯予防はやはり日々の歯磨きです！
フッ化物配合の歯磨き粉を使用することで虫歯予防につながります。
さらに、歯医者さんでより高濃度のフッ化物を塗布することで、より効果が高まります。

フッ化物をうまく使用して虫歯予防をしましょう！

原田歯科の注射は痛くない？！

こんにちは。歯科医師の亀田です。
最近、食パンブームらしいですね。
母もはまっていて、「乃が美」の食パンをおすそ分けしてもらいました。
フワフワもちもちでとっても美味しかったです。朝食が楽しみになります♪

安心して治療を受けていただくために、原田歯科クリニックでは歯科麻酔をする際、痛くない工夫をしています。

麻酔時の痛みは、大きく分けて２種類あります。
『刺痛』針を刺すときの「ちくっ」とした痛み。
『圧痛』麻酔液を入れるときの痛み。

まず、麻酔をする前に、表面麻酔をあらかじめ塗ることで注射針を入れる感覚がわからないようになっています。
患者さんからは、いつ麻酔されたかわからなかったという感想を数多くいただいています。

また、麻酔液が冷たいと、歯茎に麻酔液を入れた時に痛みを感じるので、使用前に、麻酔液を人の体温と同じくらいに温める事でその痛みを軽減させています。

さらに、歯の麻酔に使う注射針は「0.2mm」サイズの『33ゲージ』の細い針を使用しています。
糖尿病の患者さんが「インスリン投与」で使用するのが“世界で一番細い”「0.18mm」の『34ゲージ』といわれており、原田歯科クリニックの麻酔針は、その次に細いサイズを使用して、痛みを和らげる工夫をしています。

暑い日が続いていますね。夏バテには十分気を付けてください！
この前、お肉をいっぱい食べて体力をつけてきました！
ちなみに、１番好きなお肉は、北海道出身なのでラム肉です♪ジンギスカン大好きです♪

高齢者になって口腔機能が低下すると、認知機能や全身の代謝低下が起こりやすくなります。

オーラルフレイルと呼ばれる近年注目されている考え方で、口腔機能が低下することで、食への関心も低下し、体力や運動機能の低下を引き起こします。

しかし、近年の研究により口から食べものを食べることで健康維持につながるメカニズムが分かってきました。

それは、口から食べものを食べ、よく噛むことで、上咽頭の副交感神経が刺激され、全身の代謝を調節するホルモンの分泌を促進します。
さらにこの神経の活動は、嚥下反射を誘発します。
これにより、全身の代謝機能の向上だけでなく、誤嚥性肺炎などを防ぐことにもつながります。

いつまでも健康に過ごすため、口から食べものを摂るにはお口の中の状態が非常に大切になってきます。
