今日は、患者さんからよくいただくご相談のひとつ、「親知らずって抜いたほうがいいの？」についてお話します。
まず、親知らずがどんな歯かご存じでしょうか。
「親知らず」は正式には、第三大臼歯（だいさんだいきゅうし）と呼ばれる、一番奥に生えてくる永久歯です。
多くの方は、10代後半〜20代にかけて生えてきますが、まっすぐ生える方は実は少数派。
斜めや横向きに生えたり、歯ぐきに埋まったままになってしまうこともあります。
以下のようなケースでは、抜歯をおすすめすることが多いです：
•斜め・横向きに生えていて歯ぐきが腫れやすい
•手前の歯を押して歯並びを悪くするリスクがある
•歯ブラシが届かず、虫歯や歯周病になりやすい
•かみ合わせに影響が出ている
特に、親知らずの周りの歯ぐきがたびたび腫れる方は、炎症を繰り返すと顎の骨に影響を及ぼすこともあるので要注意！
すべての親知らずを抜くわけではなく、抜かなくていい場合もあるんです。
•まっすぐ正常に生えていて、かみ合わせにも問題がない
•ブラッシングがしっかりできている
•将来、ブリッジや移植のために使う可能性がある
という場合は、無理に抜く必要はありません。
当院では、レントゲンやCTを用いて、個々の状況をしっかり確認してからご提案します。
抜歯時に「痛そうで不安…」という声もよく聞きます。
親知らずの抜歯は、歯の位置や根の形によって難易度が変わるため、簡単に抜ける場合もあれば、少し腫れが出ることもあります。

Today, I’d like to talk about one of the most common questions I get from patients: “Should I get my wisdom teeth removed?”
First, do you know what wisdom teeth are?
Formally called third molars, wisdom teeth are the permanent teeth that grow in at the very back of the mouth.
Most people grow their wisdom teeth in their late teens to twenties, but those who grow straight are actually in the minority.
They can also grow diagonally or sideways, or become buried in the gums.
In the following cases, extraction is often recommended:
• The tooth is growing diagonally or sideways, which can easily cause gum swelling
• There is a risk that the tooth will push against the teeth in front, causing misalignment
• The toothbrush cannot reach the tooth, making it more susceptible to cavities and periodontal disease
• The bite is affected
In particular, if you frequently experience swelling in the gums around your wisdom teeth, repeated inflammation can affect the jaw bone, so be careful!
Not all wisdom teeth are removed, and there are cases where they do not need to be removed.
• If they are growing straight and normal, and there are no problems with your bite.
• If you brush your teeth properly.
• If you may need to use them for a bridge or transplant in the future.
In these cases, there is no need to forcefully remove them.
At our clinic, we use X-rays and CT scans to thoroughly check each individual’s situation before making a proposal.
We often hear people say, “It looks painful, I’m worried…” when extracting wisdom teeth.
The difficulty of wisdom tooth extraction varies depending on the position of the tooth and the shape of the root, so it may be easy to remove, or there may be a little swelling.

首先，你知道什么是智齿吗？
智齿正式名称为第三磨牙，是长在口腔最深处的恒牙。
大多数人会在十几岁到二十多岁时长出智齿，但长得整齐的智齿其实是少数。
智齿也可能斜着长，也可能侧着长，或者长到牙龈里。
在以下情况下，我们通常会建议拔牙：
• 智齿斜向或侧向生长，容易导致牙龈肿胀
• 用力推前方牙齿可能会挤压智齿，导致牙齿排列不齐
• 牙刷无法触及智齿，可能导致蛀牙和牙周病
• 咬合受到影响
尤其要注意，如果智齿周围的牙龈经常肿胀，反复发炎会影响颌骨，所以一定要小心！
并非所有智齿都需要拔除，有些情况下无需拔除。
• 智齿生长整齐正常，咬合正常
• 智齿刷牙正确
• 未来有可能用于牙桥或牙移植
在这种情况下，无需强行拔除。
在本诊所，我们会使用X光和CT扫描对每位患者的情况进行全面检查，然后再提出治疗方案。
拔除智齿时，经常听到有人说“看起来很疼，好担心……”。
拔除智齿的难度取决于牙齿的位置和牙根的形状，所以拔除智齿有时容易成功，有时可能会出现肿胀。
